  • ​Must have completed the first and second year programs offered by Formations Bible College 

  • Must be in a growing relationship with Jesus Christ

  • Must accept Formations Bible College's statement of faith

  • Be involved in a local church, Christian community or be open to become part of a Christian community through the school

  • Complete application form
